Patellar Tendinopathyのリハビリテーション・アプローチ その1。

さて、アメリカに帰ってきて5日あまりですが、
時差ボケ+課題が溜まって睡眠不足で、日本滞在最後に引いた風邪が治りませぬ。うぬー。

さて、提出する課題のひとつにタイトルにある「Patellar Tendinopathy(膝蓋腱傷害)のリハビリ・アプローチ」を題材にして色々調べてみました。

●Patellar Tendinopathyとは
膝蓋腱の痛みはジャンプを多用するバスケットボール、バレーボール等の競技でよく見られることから、「ジャンパーズニー」と呼ばれることもあります。…が、この非医学的名称だとQuadriceps Tendinopathy(大腿四頭筋腱傷害)やPFPS/Anterior Knee Painも含まれてしまうこともあるので、膝蓋腱に起因する痛みは総称として「膝蓋腱傷害」と呼ばれるのが好ましい、というのが最近のトレンドです。1Patellar Tendinopathyのリハビリテーション・アプローチ その1。_b0112009_09894.jpg

 - Paratentinitis (Tenosynovitis)
 - Tendinitis
 - Tendinosis

腱傷害は大まかに腱膜炎、腱炎、腱症の3つに分類され、中でも腱組織には血流が充分にない部位が多い→炎症を起こす能力に欠けている為(血流がなければ炎症は起きません)、2 炎症を起こさずに腱の変質・退化が起きるケース(= Tendinosis)が腱炎に比べて非常に多いことがここ10年程で医学界に浸透しつつあります。TendinitisとTendinosisを同時発症することもあるんですけどね。

これらの腱傷害は長期に及ぶ痛み、機能低下、そしてアスリートのパフォーマンス低下に繋がり、1,3,4 中でも膝蓋腱傷害が原因で引退を強いられるアスリートもかなり多い(53%という報告も4)のだそう。

で、Patellar Tendinopathyについて色々掘り下げて文献読んでたら結構面白くって。中でも、Patellar Tendinopathyはregion specificだ、という意見があって、腱障害が起こるのはほぼ「posterior proximal tendon at its insertion of the patella(腱の膝蓋骨付着部、後方近位部位)」に集中しているということでした。3 なんでも腱後方の繊維は前方の繊維に比べてtensile strengthが劣るので、同じ負荷がかかっても先に悲鳴を上げるのは後方なのでは…ということ、それから目から鱗だったのが「もっと言うと、前方繊維は構造的に腱のそれに酷似しているのに対して、後方繊維はどちらかというと靭帯に近い組織で作られている。これは、前方繊維は大腿四頭筋から伸びている大腿四頭筋腱の延長である(筋肉⇔骨=腱)のに対し、後方繊維は膝蓋骨と脛骨を繋ぐのみ(骨⇔骨=靭帯)である、という解剖学的違いから考えても納得がいく」(意訳ですけど)というPearson & Hussain3の言葉ですねぇ。機能別に、Anterior Tendon UnitとPosterior Ligament Unitとわけて考えてもいいのでは?ということでした。なるほどねぇ。へえええええ。

●リハビリテーション・アプローチ
さて、ひと度腱組織が病変を起こし:3,5,6
1. Decrease stiffness
2. Decrease Young's modulus
3. Increases proximal tendon cross-sectional area (CSA)
といった組織的変化の影響を受けると、load時の変形がより如実/チカラを生み出す妨げになり、7,8最悪更なる怪我に繋がる8…という結果が考えられます。腱繊維もType Iが減少、Type IIIが増加してウェーブ状に変質し、小規模の断裂が各所で起こるそうな。12

●救世主参上!?
さてそこで現れたのがEccentric Exercise!
TendinopathyにはEccentric Exerciseセヨ、というのはもう、世のAT/PTには常識なのでは?と言ってもいいくらい、ここ7年くらいで「ゴールドスタンダード」として定着しつつあります。

このEccentric Exerciseというコンセプトは元々1984年にStanish & Curwin10が提唱したもので、後にAlfredson11によって1998年に修正されています。これらのエクササイズの目的は、
 - Promote collagen fiber cross-linkage formation10,13
 - Facilitate tissue remodeling10,13
 - Decrease pain mediators in tendinopathic tendon14
 - Decrease neovascularization15
…というのが上げられます。

ではさっそくオリジナルのエクササイズの紹介を。
1984年にStanish & Curwinが提唱したモデルはEccentric Drop Squatsというもので、
 - 両足で平らな地面に立つ
 - 腰を落とすように急激に落下(Rapid deceleration)させ、
  大腿四頭筋を使って丁度太腿が地面に
  並行になる直前に減速して止める
 - 10-20回を3セット、一日に1回行う
 - minimal pain allowed: 痛みはほとんど無く行う
  (3セット目の終わりに軽い痛みを覚える程度ならOk)
 - 20回3セットが簡単に行えるようになったら、スピードを上げる
 - その後で、loadを増やして荷重を上げる

●Alfredson Model11
一方で、Alfredson Modelは
 - 片足で、25°のDecline boardの上に立つ
 - そこからゆっくりと膝を屈曲し(Slow deceleration)、膝関節が90°になるまで曲げる
 - 怪我をしていない方の足を使ってStarting positionに戻る
  (=運動自体はpurely eccentric)
 - 15回を3セット、一日に2回行う
 - この運動は多少の痛み(some pain)と共に行うべきものである。そのため、痛み無く
  できるようになったら背中にリュックを背負う形で荷重し、loadを増やす。
特に、このエクササイズを始めて1-2週間はかなりsoreになるのが『アタリマエ』なんだそう。

似ているように見えて、両足vs片足、平面vs斜面、速いvs遅い、痛み無しvs有り、スピードを上げるvs荷重を上げる、一日1回vs2回…細かく見ていくとかなり色々違いが有りますね。

●どっちがいいのさ?
…で、色々文献を掘り返して見てみました結果をまとめます。
4-5ほどのSystematic Reviewを読んでみました。

結論としては、
Limited Evidence*: Eccentric Exercise works for both short- and long-term
 - 膝の機能、痛みが大きく改善する可能性が50-70%アリ1,12,15,16
 - 手術と比較したRCTでは、「手術するのと同じだけの結果が出る」という結論に12
 - 患者の満足度・競技復帰にも大いに効果アリ1
 - adverse effectsは報告されず
副作用が無いことからも、ここまで見ただけでも手術するより先にEccentric Exerciseを試すべき、ということは言えそうですね。
*数はそこそこあるが、low-qualityな研究によるエビデンスである

あとは、具体的にStanish & Curwin vs Alfredsonを個別に考慮すると、
 - Stanish & Curwin vs Alfredsonを比較した研究は…ちょっとどちらのエクササイズも
  オリジナルなものから逸脱してしまっており(i.e. 両方のエクササイズを片足で
  やってしまっていたり、Alfredson Modelの膝の屈曲度を90°から60°に変更して
  いたり…)、『どちらのモデルも等しく効果的』という結果が出ているものの、
  これらの要素が影響している可能性があり、
  どこまで信用していいものかは定かではありません。12
 - 研究としてはAlfredson Modelのほうが遥かに研究されていて、
  concentric on a decline board vs eccentric on a decline board
  eccentric on a flat surface vs eccentric on a decline board
  などの比較研究から言えることは、効果的なのはeccentric on a decline board
  の特殊なコンビネーションのみということです。12
あとは、
 - Pulsed USやFriction massageよりもAlfredson Modelが効果的12
 - Static stretchとAlfredson Modelを組み合わせると相乗効果有り17
…なんてのもありました。

あと、もうひとつ面白い論点が、
Visnes et al12はその結論に「エクササイズをしている間、特に最初の6-8週間はエクササイズ以外のスポーツアクティビティは控えるべき」と論じています。その根拠として…
 - Visnes et al18の研究によれば、合計29人のバレーボール選手を対象にシーズン中に
  通常のトレーニング(i.e. 練習、ウェイト, etc)を行ったコントロール・グループと、
  通常のトレーニング+一日2回のAlfredson Modelをしたトリートメント・グループを
  比較した場合、12週間後にこれらのグループ間の差は見られなかったとの報告が。
  結論: シーズン中のAlfredson Modelの採用は効果無し。
 - Young et al19らは17人のバレーボール選手をAlfredson vs Stanish & Curwin Model
  の2グループに別け、シーズンが始まる前の早い段階から毎日実践。
  シーズンを終えたあと比較してみると、Alfredson Modelのほうがより優れていは
  いたものの、どちらのグループも著しく膝の機能改善が見られたとのこと。
  結論: 同じエクササイズをしても、シーズン前は効果が出る。

ということが上げられます。ふぅむ、シーズン中に選手がこなしている練習量+Eccentric Exerciseを足してしまうと、腱がオーバーロードされるだけで治癒や機能改善にはつながらないのか?練習や試合を諦めてでも、絶対にこちらのエクササイズに集中させたほうが得策なのか?でもそれって、我々にとって、選手にとって、現実的なんでしょうか?
